Chronic Disease Management

The steps below show how to set up the chronic disease features of Medchart

• choose a disease such as diabetes
• Go to Chronic Disease- Chronic diseases Names and create a new record entitled"Diabetes"

• if the disease is billable with fee item 13050 make sure you check off the "billable" box above

• Now got to Chronic Disease- Chronic Disease Items. You will need to create items to follow such as blood pressure, HDL, hemoglobin A1C etc.
• create a new item- for example hemoglobin A1c
• 

• for item name enter hemoglobin A1c
• for data to track select lab test
• for "repeat every" enter the frequency the test should be entered (e.g. every 4 months)
• in the data to track section select every occurrence of hemoglobin A1c in the lab test names (it may appear several times if you use Medinet and Pathnet)
• repeat the same procedure for other items

• Now go back to Chronic Disease- Chronic Disease Names
• in the "items to be followed..." section select all the chronic disease items to be followed.
• The final step-- for each patient go to "Chronic Disease--Chronic Disease Patient Profile" and select any chronic diseases the patient may suffer from
